For our 2022 Summer Camps, Northfield Tennis will be following and adhering to government guidelines for COVID-19, which currently include the following policies.We will update these policies if/when government guidelines are changed/updated.
Sanitizing stations will be provided and participants will be given access for frequent hand sanitizing
Participants will complete a health screening process prior to start of camp each day, and any participants with symptoms consistent with COVID will not be allowed to participate in camp
Before bringing your child to camp each day, please insure that your child does not have any of the following symptoms:
Fever (100.4) or chills
Cough
Shortness of breath or difficulty breathing
Fatigue
Muscle or body aches
Headache
New loss of taste or smell
Sore throat
Congestion or runny nose
Nausea or vomiting
Diarrhea
COVID-19 Liability Waiver
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing and mask wearing.
I further acknowledge that Northfield Tennis has put in place preventative measures to help reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Northfield Tennis can not guarantee that my child/children will not become infected with the Coronavirus/Covid-19.
I acknowledge that I will comply with all set procedures to help reduce the spread while my child/children attend tennis camp.
I hereby release and agree to hold Northfield Tennis harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act , or that may otherwise arise in any way in connection with any services received from Northfield Tennis. I understand that this release discharges Northfield Tennis from any liability or claim that I, my heirs, or any personal representatives may have against them with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Northfield Tennis. This liability waiver and release extends to Stapleton Tennis Academy LLC, DBA Northfield Tennis, with all owners, partners, and employees.
By registering for Northfield Tennis Summer Camps, I acknowledge that I have read and agree with this waiver.
